must check in 30 minutes prior to game time

U-8 coaches: Bring one (1) Misconduct Report/Score Sheet
that is completely filled out, with jersey numbers included

U-9-U19: Bring five (5) Misconduct Report/Score Sheets
that are completely filled out, with jersey numbers included

Medical release forms for every player

Hotel information for out-of-town teams staying overnight

NO guest player forms will be accepted
